Three incidents in one week show autonomous AI slipping its leash in ways that matter. This spring, a chatbot nearly put US forces on a Chinese ship after it falsely flagged the cargo as nuclear weapons components, sending aircraft up and boarding teams into position before officials caught the hallucination that one source said "almost started a war." And three researchers at Hacktron AI used Anthropic's Claude to hack OpenAI in under 72 hours, chaining two bugs to reach its internal Monorepo and collecting a $6,500 bounty; OpenAI later pulled 25% of its production engineers onto security.

Why It Matters: The through-line is that none of this took a genius attacker or a rogue superintelligence. A misconfigured test, a bad prompt, and three people with off-the-shelf subscriptions were enough.
